Abstract

Introduction: Fractures of the angle of the mandible related to third molar extraction are an infrequent complication in dentistry, but with significant clinical consequences. Objective: To investigate the risk factors, surgical techniques, diagnostic methods, and treatment options related to mandibular fractures associated with third molar extraction. Case presentation: A 27-year-old man presented with an inability to close his mouth after third molar extraction. Surgical intervention involved an extensive osteotomy and lever techniques, radiographically confirming a fracture of the mandible. Given the complexity of the case, a reconstructive surgical procedure was performed by an expert in maxillofacial surgery, achieving realignment of the fractured bone segments and allowing for the patient's functional recovery. Discussion: This analysis underscores the importance of recognizing preoperative risk factors and using appropriate surgical methods to reduce the risk of mandibular fracture during third molar extraction, highlighting the importance of detailed surgical planning in high-risk situations. Conclusions: The cause was a third molar extraction in the lower jaw, identified through panoramic radiography and CT scan. A mandibular block with Erich bars was performed. Treatment included antibiotics, anti-inflammatories, and analgesics, as well as care such as thermotherapy and maintaining good oral hygiene.
