Active management of the third stage of labor for the prevention of postpartum hemorrhage

Abstract

Postpartum hemorrhage remains one of the leading causes of maternal morbidity and mortality worldwide. This literature review, based on sources such as PubMed, ScienceDirect, SciELO, and Elsevier, addresses the need to implement comprehensive clinical strategies to optimize hemodynamic management during this period. The goal is to prevent massive bleeding, combat maternal anemia, and reduce dependence on blood transfusions. The research emphasizes the critical importance of active surveillance and early identification of risk factors. Key interventions analyzed include active management of the third stage of labor, rational and timely use of uterotonics, and volume replacement guided by clinical and laboratory parameters. Evidence gathered from scientific societies and systematic reviews shows that proactive, protocolized management not only prevents serious complications but also significantly improves clinical outcomes for the mother. Adopting this comprehensive approach represents a fundamental shift toward safer, more efficient, and patient-centered obstetric care. Its correct application not only reduces the rate of complications but also contributes to the optimization of hospital resources, consolidating a model of care that prioritizes maternal safety and well-being from an evidence-based perspective.
